1. I'd bring it back to the original call. Free kick outside the area. Yellow card to the goalkeeper.

2. Forfeit the game if he doesn't kick.

3. Original call stands. If the attacker wants to walk off the pitch without permission, yellow card him.

Probably all wrong but I'm no ref.

1. Goal is offside, so disallow the goal. Bring play back and yellow card the keeper. Restart with an indirect free kick to the defending team.

2. Tell him that he has to take the penalty. If he continues to refuse then his take will go down as a missed kick.

3. You call the striker back and tell him in no uncertain terms that he has to keep on playing. Inform the manager of the situation and write down all of this in the match report. If there was no contact, the red card will be rescinded anyway.

1. You have to question the fourth official here and see if the substitution was a legal one. Depending on what the fourth official says depends on whether the goal is allowed. If the fourth official is unsure, award the goal and just highlight all of this in the match report.

2. Tell him no.

3. First things first, stop the play and tell the teams physios to come on and check the keeper. Alert the manager of the situation and tell him he should make a substitution as the keeper is in no fit state to continue. The goal isn't awarded, restart the game with an indirect free kick to the defending team.
